What is Achalasia Cardia?

Achalasia Cardia is a pathological condition in which the esophageal narrowing or abnormality of the esophagus causes difficulty swallowing food and associated symptoms, especially difficulty swallowing food. In this pathological condition, as the esophagus becomes thin, swallowing food can be difficult or painful. Sometimes swallowing food can cause chest pain. Sometimes there is difficulty swallowing food, even if there is no pain. Sometimes swallowed food regurgitates. All these various difficulties hinder a person from leading a normal life.

Who gets Achalasia Cardia:

A survey showed that Achalasia Cardia is more common in people aged 25 to 60 years. However, beyond this age group, it is sometimes found in the young and old, and both men and women have equal rates of Achalasia Cardia.

Causes of Achalasia Cardia: The exact cause of esophageal achalasia cardia is not known, but it is believed that an abnormality or malfunction of the esophageal nerve is responsible for the disease. Autoimmune conditions, genetic factors, and, in some cases, viral infections are also responsible for this disease. Although this disease occurs in people of all ages, it is more common in people between the ages of 30 and 65.

Symptoms of Achalasia Cardia:

The most important symptom of achalasia cardia is difficulty swallowing food. Sometimes there is a feeling of food sticking in the throat or chest. Sometimes swallowed food can come out again (reurgitation of food). Along with this, some may have symptoms of acidity. Chest pain may be accompanied by chest pain when swallowing food. And if this trouble continues for a long time, the patient may lose weight due to illness. Various other disease symptoms, such as bronchopulmonary symptoms, etc., may appear.

Complications of Achalasia Cardia:

Sometimes the swallowed food comes out again (regularisation of food), so sometimes this food can enter the trachea from the esophagus. Sometimes this food can enter the lungs through the trachea and cause complications. For example, pneumonia, lung infections, and even esophageal cancer can be caused by achalasia cardia.

Diagnosis of Achalasia Cardia:

A doctor can diagnose Achalasia Cardia in various ways. The disease can be diagnosed by reviewing the patient’s medical history, physical examination, and some pathological tests. Esophageal manometry is a pathological test to measure whether or not the oesophagus has contracted. A barium swallow or upper endoscopy can also give an idea about the actual condition of the oesophagus.

Management of Achalasia Cardia:

If a person is suffering from this disease, he needs to follow some lifestyle changes, especially diet-related rules. Then the patient’s life will be easier despite the presence of the disease.
For example-
1) The food should be chewed and liquidised in the mouth as much as possible.
2) Do not eat a lot of food at once, but eat little by little.
3) Foods that increase the disease-related suffering of the person should be avoided.
4) Eat while sitting upright.

Is Achalasia Cardia fatal?

Usually, no one dies from this Achalasia Cardia disease. But if you have suffered from the problem for a long time, it is possible to avoid serious situations by changing your lifestyle or diet. But be aware, because those who have Achalasia Cardia have a very high risk of cancer of the oesophagus.

Types of Achalasia Cardia:

Achalasia cardia is generally seen in three types:
Type 1: (Classic Achalasia): Here the problem of Achalasia Cardia arises from the problem of lower esophageal sphincter (LES) relaxation.
Type 2 (Achalasia with Compression): Imbalance of contraction and expansion along the entire length of the oesophagus and incomplete LES relaxation is observed.
Type III: (Spastic Achalasia or Vigorous Achalasia) Incomplete LES relaxation with lack of contraction and dilation of the oesophagus and accompanying premature contraction problems are present.

Treatment of Achalasia Cardia:

Achalasia cardia is treated in different ways in allopathic treatment. Non-surgical methods include medications, balloon dilation, etc. All of these procedures have one goal: to help the patient’s LES (Lower Esophageal Sphincter) expand as much as possible due to the contraction that prevents food from entering the stomach.

Balloon dilation: Keeping the patient in a light unconscious state, a specially designed balloon-like tube is inserted into the LES at the lower end of the esophagus, and the balloon is then inflated so that the area expands slightly and the patient can swallow food.

Medication: The patient with Achalasia Cardia is treated by taking some special medicines, if necessary, the patient is also treated by blind injection.
Laparoscopic Esophagimyotomy: The surgical operation of Achalasia Cardia is called Laparoscopic Esophagimyotomy or Laparoscopic Heller Myotomy. In this procedure, a small incision is made on the side of the LES part of the esophagus, thereby widening that part of the esophagus.

POEM: (Peroral Endoscopic Myotomy) Myotomy is performed through an endoscopic procedure through the mouth.

Esophagectomy: Removal of the entire esophagus is usually done if there is a cancerous cause.

Complications in the treatment of Achalasia Cardia:

Even after treating Achalasia Cardia with all the above treatment methods, some complications may remain. such as-
1) A surgical operation can cause a hole in the esophagus.
2) Many times the treatment is not successful and the disease comes back again.

Achalasia Cardia Awareness:

It should be remembered that whatever type of Achalasia Cardia is treated, it is always palliative. Achalasia Cardia is usually not completely cured with these treatments, and most of the time the disease comes back.

Personal Experience in Achalasia Cardia Treatment:

Last year, I treated an 80 years old man for Achalasia Cardia. The patient’s problem was dysphagia. The patient had difficulty swallowing food. I gave him the medicine Lacases 200 strength for similar symptoms, and the patient got better. But after a long time, the disease would return to some extent. Then again, if a new dose of medicine was applied to him, the patient would be well again for a few days. However the patient did not fully recover.
